Good Faith Estimate Notice

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

If you are not using insurance or plan to pay out-of-pocket for services, you are entitled to receive an estimate of the expected charges for therapy services before services begin.

You may request a Good Faith Estimate before scheduling services or at any time during treatment.
